The survey is six yes/no questions with an explainer if you don't know, it should take less than a minute..

Also, I had asked Sarah if she had anything to include in the newsletter and I forgot to encourage you on her behalf:
Remind them not to freak out over OPD, that it doesn't matter if you don't get every question correct, in fact you can get every question wrong and nothing will happen as long as you have gone through the process of doing it.
